In short:
A 35-year-old man died on Thursday after being shot multiple times by three police officers outside Manning Base Hospital.
Police Northern Region Commander David Waddell says the man was killed after multiple attempts to de-escalate the situation failed.
What's next?
Police will investigate why the man was at the hospital while armed.
NSW Police has confirmed officers tasered a man armed with a revolver before shooting him dead outside a hospital on the NSW Mid North Coast.
A critical incident investigation was launched by the Manning-Great Lakes Police District on Thursday, after a 35-year-old man was shot by officers in the car park of Manning Base Hospital in Taree.
Police Northern Region Commander Assistant Commissioner David Waddell said responding officers used a taser after the man pointed his weapon at police.

"Police called on the male to drop the firearm, he then pointed that firearm at them," Assistant Commissioner Waddell said.
"They eventually deployed a taser before fatally shooting him at the scene.
"We're still investigating whether he actually fired any shots at police at that location."

Police had earlier been called to a home on Summerville Street in Wingham at 12:20pm on Thursday to conduct a welfare check for the man.
Five minutes later, police received reports that the man had travelled to a neighbouring address and discharged a firearm into the air.
Assistant Commissioner Waddell said police officers travelled to Manning Base Hospital after receiving reports he was being driven to the hospital by a woman.
"We don't know why he went to hospital, that's subject to the critical incident investigation," Assistant Commissioner Waddell said.
"We're obviously speaking to plenty of people as part of that, including that female."

Arriving at the hospital at 12:55pm, the man was met by four police officers including three junior officers.
After a stand-off lasting several minutes, he was shot multiple times by police after refusing to lower his weapon.
"One discharged a taser and the other three [police officers] discharged their firearms," Assistant Commissioner Waddell said.
"They are fairly junior police, three of them were first class constables and the other was a leading senior constable.
"They will obviously have all those welfare support services wrapped around them, [and] will be interviewed as part of the critical incident investigation."
A critical incident investigation will be led by the New South Wales Homicide Squad, who will prepare a report for the coroner.
It comes after five critical incident investigations were launched by police in three weeks across the Mid North Coast of NSW.
They include a 32-year-old man who was shot by police at a sporting oval in Grafton, after allegedly threatening officers with a knife.
